This is a dental discount plan and not dental insurance

Family members cannot be substituted for other family members
Enrollees must be stated at the time of enrollment
Discounts do not apply to take home whitening products
Annual enrollment fee must be paid in full on or before first dental visit
This plan is NON-REFUNDABLE and is for one year from acceptance date
Any unused benefits cannot be carried over to another year
Patient's portion of bill is due the day of service. Failure to pay at time of service, failed or broken appointments without a 24-hour notice, voids your plan
